Under the updated guidelines, Form 5 is for candidates with a single disability, while Form 6 is meant for those with multiple disabilities.
The Staff Selection Commission (SSC) has updated the disability certificate formats as per the latest guidelines from the Department of Empowerment of Persons with Disabilities (DEPwD).
Under the updated system, Form 5 will now be issued to candidates with a single disability, while Form 6 will be used for those with multiple disabilities. These updated formats replace the earlier three forms — 5, 6, and 7 — that were mentioned in previous SSC notifications.
According to the official notice, for exams whose notifications were issued after October 16, 2024, and are still in progress, candidates can submit their disability certificates in either the newly revised formats (Form V and Form VI as per the DEPwD guidelines) or the previously used formats.
In addition, SSC has restored the ‘own scribe’ option for candidates with benchmark disabilities (PwBD) for all examinations announced on or before December 31, in line with the recent clarifications issued by the DEPwD.
